Piekeren is slecht voor je hart!
June 9, 2008 on 7:21 pm | In Stress, Burnout & Depressie |Mensen die zich gemakkelijk zorgen maken en daarnaast vrij geremd zijn, de zogenaamde type D-persoonlijkheden, maken na een eerste hartinfarct minstens vier keer zoveel kans op een tweede hartaanval dan meer zorgeloze en open types. Tot die conclusie kwam psycholoog prof. dr. Johan Denollet, verbonden aan de dienst cardiologie in het UZA.
Aanzet tot onderzoek
Denollet werkte een aantal jaar als psycholoog op de cardiale revalidatie van het UZA. ‘Het viel mij toen op dat patiënten heel verschillend reageerden op hun hartinfarct. Sommigen stapten er vrij gemakkelijk over, terwijl anderen het er heel moeilijk mee hadden en bijkomende lichamelijke klachten ontwikkelden, zoals vermoeidheid of ademnood. Zo groeide het idee om het verband tussen iemands persoonlijkheid en het risico op nieuwe hartproblemen te onderzoeken’, vertelt Denollet.
Resultaten
Het onderzoek loopt intussen al een tiental jaar. In totaal verleenden 1.175 patiënten hun medewerking. Uit de resultaten blijkt dat type D-persoonlijkheden een sterk verhoogd risico hebben op een tweede hartinfarct. Chronische stress heeft een negatieve invloed op het immuunsysteem, wat op zijn beurt kan leiden tot een hoge bloeddruk en problemen met de hart- en bloedvaten.
‘Om extra risico te lopen moeten mensen beide persoonlijkheidskenmerken vertonen’, nuanceert Denollet. ‘Enerzijds gemakkelijk negatieve gevoelens ervaren, anderzijds gesloten en geremd zijn. Het zijn vooral die types die door de jaren heen grote spanningen opbouwen en hier emotionele hinder van ondervinden. Mensen die zich gauw zorgen maken, maar hierover gemakkelijk praten met hun omgeving, lopen het verhoogde risico niet. Hetzelfde geldt voor personen die wel gesloten zijn, maar zich goed in hun vel voelen.’ Uit het onderzoek is gebleken dat de genoemde persoonlijkheidskenmerken een minstens even
even grote risicofactor vormen als bijvoorbeeld een te hoog cholesterolgehalte of hoge bloeddruk.
Test
Intussen ontwierp Denollet een test die aan de hand van veertien vragen nagaat of iemand een type D-persoonlijkheid heeft. Het Amerikaanse magazine Newsweek wijdde een artikel aan het onderzoek en prompt dook de test op tal van websites op.
Wat als ik type D-persoonlijkheid heb?
‘Als mensen op basis van de test vaststellen dat ze een type D-persoonlijkheid hebben, moeten ze niet panikeren’, beklemtoont Denollet. ‘Ongeveer 25 procent van de bevolking behoort tot deze categorie, maar lang niet iedereen ervaart dit als een probleem. Bovendien spreken we over een risico, wat wil zeggen dat een groot aantal geen tweede hartinfarct krijgt.’ Als mensen beseffen dat hun piekergedrag en geremdheid wel een probleem vormen, kan de test een duwtje in de rug zijn om een en ander in hun leven te veranderen. ‘In eerste instantie raad ik die mensen aan om over hun probleem te praten met een vertrouwenspersoon’, zegt Denollet. ‘Daarnaast zouden ze zich extra moeten inspannen om andere risicofactoren als roken of een te hoog cholesterolgehalte onder controle te houden. Als er bijkomende lichamelijke klachten zijn - bijvoorbeeld gewichtstoename of een drukkend gevoel in de borst -, melden ze dat best zo snel mogelijk aan hun arts. We stellen vast dat type D-persoonlijkheden daar vaak lang mee wachten. Tenslotte kan het voor sommigen nuttig zijn psychologische hulp in te schakelen. Zo kunnen ze leren anders om te gaan met stress en moeilijke situaties.’ Of mensen met een type D-persoonlijkheid ook gemakkelijker een eerste hartinfarct ontwikkelen, is niet bekend. Denollet voert hierrond verder onderzoek.
